Care Compass Network has partnered with the VNS of Ithaca to provide Health Coach Training to eligible employees of hospital systems and community-based organization who are contracted to participate in the Care Transition (2biv) Project. A minimum number of 2 attendees is required to hold this training. Registration is required. Registrants must become active Health Coaches after receiving training. Who should attend the training? The Project Champion for each organization is strongly encouraged to attend. Eligible Health Coaches selected by the Project Champion at each organization and who will be preforming the facility visit, the home visits and the three follow-up phone calls. The role of the Health Coach in the CCN Care Transition Intervention Model is to coach Medicaid Members and family caregivers to promote skill-transfer of self-care skills and have caregiver develop skills and confidence in four key areas, known in the CTI project as “pillars.” These are: 1. Medication self-management 2. Primary care provider or specialist follow-up 3. Patient understanding of “red flag” indicators of worsening condition and appropriate next steps 4. Use of a patient-centered health record that helps guide patients through the care process http://caretransition.org/ Once a patient is referred to a home care agency and/or a community agency for health coaching services, the transition coach visits patient in the hospital to describe the intervention and invite the patient to participate. The visit is followed by a home visit and three follow-up phone calls over a four-week period. Training Format VNS of Ithaca, shall perform interactive in-person classroom training sessions for up to 16 participants for an 8-hour training. What will I learn? This training opportunity will train individuals in coaching patients build their confidence and competence in the four conceptual areas, or “pillars: medication self-management, use of a patient-centered health record, primary care and specialist follow-up and knowledge of red flags of their condition and how to respond. Lecture will be provided on each of the four pillars. A break out session will follow each section on the pillar affording participants practice time in the skill area. Who teaches the program? The program is led by Sue Ellen Stuart, (BSN, MS) CEO of VNS of Ithaca and staff from the VNS of Ithaca.
